Once you accept your proof, you will be provided a 6 digit design number. Copy that number & come back to Etsy. 6. Paste that number into the box under "Add Personalization" 7. From the drop-down, select "I followed the steps" 8. Finish checkout on Etsy Want to leave off a date or other field? Just leave that box empty in the form fill.
