I've not been able to find another example of this so I'm going to call it scarce. Equitable Life of Iowa is still in business but this policy cover probably dates from the 1930's to 1940's perhaps just based on style & materials of the cover which is similar to bank & savings account books, leather like material embossed in this case with the headquarters building for Equitable Life in Des Moines, Iowa that opened in 1924 is located at 604 Locust St & is now a residential apartment building.
