In New Amerika an ongoing conflict between rival corporations resulted in the Techno-Industrial Civil Wars, where the need to stay ahead of recent cybernetics and bioengineering developments has made espionage, murder and other nefarious deeds all too common. Attempting to provide order, a Secret Games Commission regulates contract and territorial disputes among corporations who agreed to bring forth their own "representative" to fight in a closed-off arena.

Of course, each company had to ensure they had the best warrior available for the duel, so they used their research experience to create some of the deadliest creatures known to man called Bio F.R.E.A.K.S., short for Biological Flying Robotic Enhanced Armored Killing Synthoids. Synthoids have the ability to re-grow tissue. Retros have cybernetic limbs. Dozers are completely machine-like in nature, save for their brain.

The eight participants hailed from the largest corporations in New Amerika: Bullzeye, a Synthoid soldier, carries a grenade launcher; Delta, a female Synthoid known for her speed; Minatek, a Dozer resembling a Minotaur; Psyclown, a hammer-wielding Synthoid dressed like a clown; Purge, a Retro with arms that fire napalm canisters; Sabotage, an unclassified female mercenary; ZipperHead, a gray Retro soldier with a spiked ball and buzz saw for arms; and Ssapo, a toad-like mutant with razor-sharp teeth.

Players choose one of eight creatures and challenge the remaining opponents, including the two boss characters Mutilator and Clonus. The game initially offers eight 3D fight arenas, each containing hazards such as toxic waste, lava, rotating blades and even meat grinders. A training arena is available to learn the more than twenty special moves for each character. Arenas also feature a tier system allowing players to fly across ledges or perform mid-air attacks using jet packs.

Bio F.R.E.A.K.S. carries a mature rating for its depiction of fatalities, mutilations, and other displays of graphic violence. The game supports the ~Rumble Pak for vibration feedback as well as the ~Controller Pak to save data.
